We can help you understand the aged care system and discover the range of home care services, for which you could be eligible.

We can remove the worry, and help you easily understand your choices. Then, we can ensure your chosen aged care services are delivered to you at home. Helping you stay in your own home and in control of your life.

Services available 

 

While we offer our services on a competitive privately-funded basis, you may be eligible for a government subsidy, such as the Commonwealth Home Support Program or a Home Care Package, to assist with the costs of these as part of the Australian aged care system.

 

Getting started 

 

While we offer our home care services on a competitive privately-funded basis, you may be eligible for a government subsidy to assist with their cost, through My Aged Care. We can help you access any government subsidy, for which you may be eligible by connecting you with My Aged Care and attend any in-home assessments with you if you would like. Find out more about assessment and eligibility.

We celebrate diversity and offer translating and interpreting services and AUSLAN interpreter services. We will respect your privacy. We will take all reasonable precautions to protect your information and will not disclose this to anyone unless you give us your permission (or we are required to do so, by law). We are continually looking for ways to improve our service and will always value your feedback.

 

Our Service Provider Commitment 

 

After you have chosen UCWB as your service provider, we will partner with you to create a service plan outlining what services will be delivered and how much they will cost. Each year, we will review your plan, with you, to ensure the services you receive are still meeting your goals and providing the support that you need. However, if at any stage you would like to review your plan, you are welcome to contact our Customer Contact Team to discuss the options available to you. 

We will ensure you receive accurate information, advice and ongoing support. With your knowledge and permission, we may also work with your family, carers or doctors to ensure your needs are being looked after. However, we will never make a decision or agree on services from individual agencies or organisations on your behalf. You will always maintain control.
