A step-by-step guide to getting started

Step 1: Contact us

Call our Customer Contact Centre on 8245 7196. We can connect you with My Aged Care as the first step towards receiving home care services.

Step 2: Phone assessment

My Aged Care will ask you some questions over the phone so they can determine the best way to meet your needs.

If you are assessed as requiring basic-level support, you will be referred to the Regional Assessment Service (RAS) who will assess your suitability to receive home care services under the Commonwealth Home Care Program (CHSP).

Step 3: Face-to-face assessment

The RAS will visit you for a face-to-face assessment. Following your assessment, you will receive a letter and your home CHSP referral code.

Step 3: Activate your referral code

If you would like for UCWB to be your service provider, please call our Customer Contact Centre with your referral code. Alternatively, you can ask your RAS assessor to provide us with the code on your behalf.

Step 4: Create a service plan

After you have appointed UCWB as your service provider, we will visit you to develop your home care services plan, in partnership with you.

The services will be based on the needs identified in your RAS assessment and the funding amount of your CHSP.

Step 5: Start your services

Your home care services will commence.
