Home Care Packages: Getting started

A step-by-step guide to getting started with a Home Care Package. 

Step 1: Contact us

Call our Customer Contact Centre on 8245 7196. We can connect you with My Aged Care as the first step towards receiving home care services

Step 2: Phone assessment

My Aged Care will ask you some questions over the phone so they can determine the best way to meet your needs.

If you are assessed as requiring a higher level of support, you will be referred to the Aged Care Assessment Team (ACAT) who will assess your suitability to receive home care services with your own Home Care Package (HCP).

Step 3: Face-to-face assessment

The ACAT will visit you for a face-to-face assessment.

Following your assessment, you will receive a letter from My Aged Care advising the level of support, for which you have been ‘approved’.

Your ‘approval’ will be placed in a national queue and there will be a waiting period before you are ‘assigned’ a home care package.

While you wait, you can access our services at a fee, if you require. 

Step 4: Active your referral code

When you have been ‘assigned’ a Home Care Package, you will receive a letter My Aged Care with your referral code.

If you would like UCWB to be your service provider, please call our Customer Contact Centre with your referral code.

Step 5: Create a plan

We will visit you to develop your own home care services plan. The services will be based on the needs identified in your ACAT assessment and the funding amount of your Home Care Package.

Step 6: Start your services

Your home care services will commence.

Step 7: Review

Every six months, we will work with you to review your plan and budget.

As your needs change, and you perhaps need a higher level home care package, we will support you to be reassessed by My Aged Care.
