Support Coordination helps you understand and use your NDIS plan by connecting you with the right services and supports to achieve your goals. Support Coordination is especially helpful if you’re new to the NDIS or managing multiple supports.

What is Specialist Support Coordination?

Specialist Support Coordination provides a higher level of assistance for people with more complex needs that require more hands-on help, assisting to manage the most challenging circumstances and ensure all services work together effectively.

We start by getting to know you – your goals, your challenges, and the types of supports that will work best for you. From there, we provide practical, hands-on support to help you make the most of your NDIS plan.

Support Coordination

Together we will:

  • Understand your plan and funding.
  • Connect with the right service providers and community supports.
  • Build the skills to coordinate your supports independently.
  • Navigate issues or changes and advocate for your needs.
  • Help you set and work toward your personal goals.
  • Assist with budgeting and managing your plan funds.

Specialist Support Coordination

Our specialist team will support you by:

  • Navigating complex systems and managing multiple providers.
  • Coordinating supports during high-risk or crisis situations.
  • Addressing barriers that make it harder to access or manage services.
  • Providing strong advocacy to ensure your needs and rights are respected.
  • Supporting you through significant life changes like hospital discharge or moving homes.
  • Working closely with health, housing, and community services.

NDIS Recovery Coaching & Mental Health Support Coordination

Our lived experience team will:

  • Help you navigate the NDIS with support grounded in lived experience.
  • Explain your plan so it’s clearer and easier to understand and use.
  • Explore what matters most to you and what you want for your life.
  • Connect you with supports and services that genuinely fit your needs and goals.
  • Reduce the overwhelm of systems and processes by breaking things down.
  • Support your mental and emotional wellbeing through this journey.

These NDIS funded services supports are part of the Capacity Building section of your plan and need to be included when your plan is created or reviewed in order for you to access them.

You, your supports, a loved one, friend, or trusted person can contact our friendly team by phone or email, or complete the online intake form via the button below. We’re here to listen and guide you through the next steps.

Initial Intake

We’ll ask a few quick questions about you, your goals, and your situation. This helps us match you with the Support Coordinator best suited to your needs and experience. To get started, we’ll invite you to sign a service agreement and consent forms so we can begin working together.

Meet Your Coordinator

Your allocated Support Coordinator will meet with you at our Bowden office, your home, or another convenient community location of your choosing. During this initial face to face meeting, you’ll work together to develop a personalised support plan tailored to your unique goals.

Ongoing Support

Your Support Coordinator will guide you through understanding your NDIS plan and funding, helping you use your supports effectively throughout your plan period. They’ll be there to answer questions, coordinate services, and support you every step of the way.

Frequently Asked Questions (FAQs)

Do I need Support Coordination funding in my NDIS plan to access UCWB services?

Yes, you must have Support Coordination or Specialist Support Coordination included in your plan to receive our services. If you don’t currently have this funding, we can help you understand how to request it in your next plan review or speak with your NDIS planner.

Absolutely. Just contact us, and we can guide you through the transition process.

We aim to provide consistency, but if changes are needed, we’ll work with you to ensure a smooth transition.

The frequency of meetings depends on your funding, individual needs and preferences. We tailor our support to what works best for you.

Yes! We can help prepare for your plan reviews and provide information to ensure your supports meet your goals.

Specialist Support Coordination is designed for participants with higher needs and more complex situations, requiring a more intensive level of support. It will be written in your plan if you have been funded for Support Coordination or Specialist Support Coordination.

Yes, UCWB is fully registered and experienced in working across a wide range of participant needs.

We offer flexible support and can meet you at our office, your home, or a community location.
