After two years in a private residential aged care home following the amputation of both of his legs, Trevor decided he wanted to return to living in his own home.
Trevor, who is in his 80s, managed to devise his “escape” with the help of one of UnitingCare Wesley Bowden’s (UCWB) Home Care Packages (HCP) and is much happier back in the home where he lived for 53 years and were his children grew up.
Through his HCP, Trevor has been able to resume living independently, shopping and cooking for himself, and attending appointments including a trip to the pub once a week with a few mates.
“A weight lifted when I was eventually able to talk to my UCWB case manager and then all of these things happened to help me at home.” Trevor
A UCWB case manager was able to help Trevor navigate his way through the complicated pathway of getting out of the nursing home and back to his own home with the help of an ACAT assessment which approved his eligibility for assistance to live at home.
“A weight lifted when I was eventually able to talk to my UCWB case manager and then all of these things happened to help me at home,” Trevor says.
He was able to get regular assistance with showering, cleaning, gardening and home maintenance.
UCWB’s case manager was also able to help Trevor’s wife Anne, obtain a Consumer Director Respire Care package to provide in home services such as a shower chair, commode and assistance to make modifications to the house and garden to ensure it is safe.
A mate of Trevor’s built a workbench for the retired engineer allowing him to “tinker’’ with things.
“It’s my home and I have certain things here I like to play with,” Trevor says.
*Trevor sadly passed away in late 2017.
